Atlanta Healthcare Systems Continue Major Expansions Amid Rising Construction Costs

Atlanta's healthcare systems are pushing forward with significant real estate expansions, investing hundreds of millions in new facilities despite a challenging economic landscape marked by escalating construction costs and high interest rates. Piedmont Healthcare is committing nearly $600 million to a new relocation hospital in the metro area, while Atrium Health and Morehouse School of Medicine secured approval for a $700 million teaching hospital and care network targeting Central and South Fulton County.

This expansion is driven by a growing aging population and a strategic shift towards hyperlocalized outpatient care, aiming to provide top-quality services within five miles of residential communities. However, developers face substantial hurdles. Construction costs for medical facilities have surged from approximately $370 per square foot in 2020 to between $439 and $455 per square foot in 2026. This, combined with cuts to federal reimbursements and rising operating costs, has led to tighter hospital profit margins.

The increased development costs make it difficult for new medical office buildings to achieve rents necessary for financial viability, creating a nearly $10 per square foot gap between existing and new space. Consequently, new MOB deliveries are projected to decline by 26% in 2026. Despite these headwinds, overall spending on hospital and clinic construction is expected to jump 11.6% this year, signaling a continued, albeit more complex, development pipeline for healthcare real estate investors.
